Diamond Geriatrics can be your eyes and ears: If you have a loved one in a nursing home, assisted living residence, or other care facility or senior’s housing location, we can help make sure they are getting the best care and services possible.
Monitoring:
We will visit your mom or dad on a regular basis for you if you are unable, traveling, or out of town. We will become familiar with their health issues, care concerns, personality, and needs, and then monitor the facility and your relative to see that your concerns and their needs are being met. We will go at different times, we will watch the companion you have hired, we will spend time with your mum or dad.
Attending Care Conferences:
Care conferences are the place and time to make sure the care being provided is what you had expected. It is also the time where the facility gets to know your expectations. We can attend those conferences with you, or for you if you are not able to do so.
Advocacy:
If you have some concerns we will meet with the staff with you or on your behalf to make address issues that have come up. Sometimes this can mean helping you to understand the care better, sometimes it means helping the facility understand you or your loved one better, and sometimes it means putting on some pressure to see that changes are made.
Adjustment:
Adjusting to care in a facility can be a long process. It is an emotional one for the resident and family—some people describe it as a roller coaster. For family members especially there are concerns about whether the care is adequate, whether the staff is skilled enough to do what they should be doing. For the facility, it is a process of learning who this resident is, and being able to respond to them in an appropriate way. We can help your relative through that period, and we can help you with teaching the facility about the person who is now in their care.
