Getting Started: Contacting Your Local Health Services
If you are looking for publicly funded services, these are the numbers to call in your area. When you call, you need to tell them that you are looking for services for a senior in their home, or you are looking for nursing homes, assisted living, or other supportive housing. They will want to know the address of the person needing care. Then they will put you through to intake, where intake will ask you some information, and hopefully set you up with a case manager, who will come to the house to do the assessment.
If someone is in hospital, assessments are done in hospital, and services can be arranged from hospital.
