Telephone consultations from Diamond Geriatrics can be an effective way for you to begin to solve some of the concerns you are dealing with. For long distance caregivers, or seniors or caregivers in areas where consultations and Geriatric Care Managers are not available, it is an effective way to receive some help.
This can also be a way for you to set up a Care Management Agreement for intervention or monitoring with your loved one if you are not able to come in for an appointment. For example, if you are an out of town caregiver, and have some concerns about your relative's care or condition in a nursing home or assisted living facility, you can arrange for a Care Manager to investigate, intervene, and/ or monitor.
How Does It Work
Telephone Consultations are prepaid through your credit card or through PayPal. To start off, call, fax, or email Diamond Geriatrics to set up a time for a consultation and choose the length of time your would like . There is a minimum one hour fee, at C$95.00 per hour.
After we have finalized the appointment time and length, we request that you send an email to confirm the details (see below).
We will then process your credit card or PayPal account by the fee amount. Once the credit is approved, one of the Care Managers from Diamond Geriatrics will phone you at the agreed upon time. If you do not answer the phone within half an hour, and three callbacks if the first time you do not answer, it will be considered a missed appointment and charged at the full rate.
If you wish to cancel an appointment, in order to receive a refund, please give us twenty four hours notice.
